Steps to Transfer Medical Records

We are excited to open the doors to my new concierge practice on Monday, September 18th. Please know that I have access to your medical records, however, I would like to have your records formally transferred to my office.

Importantly, please select one of the following options:

1. Click to complete Heritage Valley Patient Authorization for requesting medical records and mail to:

OR fax to 1-844-372-1011

OR drop off in the drop box in the Medical Records/HIM department at Heritage Valley Beaver hospital (1st floor near entrance D), Heritage Valley Sewickley hospital (4th floor), or Heritage Valley Sewickley, Kennedy Campus (1st floor near main entrance)

Note:

• Required elements with asterisk MUST be completed
• In the “Purpose of Disclosure” section, select “Continued Care”
• In the “Type of Information” section, select “Other” and specify “Dr. Hogan Medical Records”
• In the “Date(s) of Treatment” section, specify “2021 to current”
• In the “To Receive” section, specify “Dr. Hogan,” include his address, phone number, and the following fax number: 1-833-448-3280

2. Electronically request your Dr. Hogan medical records:

and select “Request Your Medical Records Online Here”
